Public health advice during a heat wave often includes reminders to drink water, watch for heat stroke, and check on those with heart or lung conditions. This advice saves lives but overlooks a crucial aspect impacting millions of older Americans: chronic pain.
A recent study by public health professor Kai Zhang and colleagues examined over 35,000 adults above age 50 over a span of twenty years. They compared participants’ chronic pain reports with the temperature history in their neighborhoods. The study aimed to understand if living with extreme heat or cold affects whether pain disrupts daily lives.
Understanding High-Impact Pain
Researchers define high-impact chronic pain as pain lasting over three months that restricts a person’s ability to work, socialize, and take care of themselves. This pain significantly influences decisions for adults over 50. It often impedes their ability to stand, carry groceries, or sleep, threatening their independence.
Temperature influences the body in pain-inducing ways. Cold increases pain sensitivity and stiffens joints. Heat contributes to discomfort through dehydration, disrupted sleep, and by limiting daily activities. For instance, hot conditions may lead to decreased physical activity, causing joint stiffness and weak muscles.
Extreme heat’s danger often lies in hot nights without relief, leaving the body unable to recover.
Research Findings
The study analyzed when neighborhoods experienced extreme temperatures. For example, a day reaching 84 degrees Fahrenheit in Maine ranks among the hottest 5% there over the past decade. In Houston, temperatures must hit 96 F to be considered extreme.
The research discovered two key findings. Those living through many cold days reported increased high-impact pain, though single cold spells had less effect. Adapting to persistent cold proved difficult.
Heat’s impact varied by geography and wealth. Urban residents showed no link to new pain from sustained heat. However, rural residents, often spending more time outdoors or in older homes, demonstrated a stronger connection. Additionally, poorer older adults were more likely to develop high-impact pain, possibly due to limited access to air conditioning and healthcare. Wealthier individuals with better resources reported less new chronic pain.

Rising global temperatures mean extreme heat exposure is likely to increase. For instance, Phoenix experienced 39 consecutive days in 2024 with no nighttime relief below 90 F.
