Chili Pepper Consumption and Cancer Risk: Exploring the Connection

People consuming high amounts of chili peppers might face increased risks of certain digestive cancers, especially esophageal cancer, based on a review of existing research. Despite these findings, experts suggest they should be interpreted with caution.
Researchers analyzed 14 observational studies involving more than 11,000 participants, including over 5,000 diagnosed with gastrointestinal cancers. The analysis revealed that individuals with the highest chili pepper intake had a 64% higher likelihood of developing gastrointestinal cancers compared to those with the lowest intake. Esophageal cancer showed the strongest association. Participants in the highest consumption category were nearly three times more likely to develop esophageal cancer compared to the lowest intake group.
The findings for other digestive cancers were less clear. The review did not find significant increases in the risk of stomach or colorectal cancer. Though stomach cancer showed a trend towards higher risk among heavy chili pepper consumers, it did not reach statistical significance.
“At this time, the research does not show sufficient evidence from a gastroenterological standpoint to support telling healthy individuals to stop consuming spicy foods,” Dr. Edmundo Rodriguez-Frias told Newsweek. He is a medical contributor at Full of Life, Marlton, New Jersey.
The findings stem from observational studies, which can identify possible links between chili pepper consumption and cancer risk but cannot determine causality. According to Rodriguez-Frias, studying chili pepper intake poses challenges due to varying definitions of high consumption and reliance on participants accurately reporting their eating habits.
Chili peppers contain capsaicin, responsible for their spiciness. Capsaicin’s effect on cancer risk has been debated as laboratory studies have produced mixed results. While some research indicates potential anti-inflammatory effects, others suggest possible pro-inflammatory effects.
Rodriguez-Frias explained that chronic irritation’s association with esophageal cancer has been explored biologically. Yet, there is little conclusive evidence linking capsaicin itself to esophageal cancer in humans. Capsaicin exhibits both anti-inflammatory and pro-inflammatory properties depending on the context.
Regional differences were noted. Studies from Asia, Africa, and North America generally reported higher cancer risks among those with high chili pepper consumption. Conversely, research from Europe and South America showed either no increased risk or lower risks.
Several factors might explain these differences, including variations in chili pepper intake, cooking methods, pepper varieties, genetics, smoking habits, alcohol consumption, and dietary patterns.
Repeated exposure to very spicy foods could contribute to esophageal lining irritation in susceptible individuals. Capsaicin activates TRPV1 receptors, leading to the burning sensation of spicy foods. These mechanisms, however, remain hypothetical and unproven.
Rodriguez-Frias highlighted that for patients, the concern might be whether spicy foods worsen symptoms like heartburn or gastroesophageal reflux disease (GERD). Controlling symptoms, such as limiting spicy foods when heartburn worsens or GERD is exacerbated, can improve quality of life.
Chronic acid reflux is a known risk factor for esophageal cancer. Rodriguez-Frias advises focusing cancer prevention efforts on risk factors with more substantial evidence rather than solely avoiding spicy foods. Strategies include avoiding tobacco smoking, limiting excessive alcohol use, maintaining a healthy weight, managing reflux symptoms, and seeking medical attention for persistent swallowing difficulties, unexplained weight loss, or ongoing heartburn symptoms.
More long-term studies are needed to determine whether chili peppers contribute directly to cancer development or if observed associations are influenced by lifestyle and environmental factors.
