Americans Face Severe Cyclospora Season as Thousands Fall Ill

Americans are dealing with an unusually intense cyclospora season. Thousands of illnesses are reported nationwide. Health officials are working to identify contaminated foods linked to multiple outbreaks. The Centers for Disease Control and Prevention (CDC) noted receiving 4,173 lab-confirmed domestic cases since May 1. There are thousands more illnesses under review. This makes 2026 the largest cyclospora season recorded. As cases rise, many question its dangers, when medical care is needed, and if the parasite can be fatal.

Understanding Cyclospora

Cyclospora is a microscopic parasite causing cyclosporiasis. This intestinal illness includes prolonged watery diarrhea, stomach cramps, fatigue, and loss of appetite. Cases usually increase during spring and summer, but this year sees an extraordinary surge. Federal investigators are studying multiple outbreaks. One large multistate outbreak links to iceberg lettuce from central Mexico. On July 17, Taylor Farms recalled certain iceberg lettuce products. The CDC has confirmed 4,173 cyclospora cases, with about 7,400 additional unconfirmed cases. Out of the confirmed cases, 308 individuals have been hospitalized, approximately 7.38 percent. Fortunately, no deaths have been reported. State officials have noted higher cyclospora case numbers.

For example, Michigan reported 7,664 cases, with 160 hospitalizations, or 2.08 percent. In Oklahoma, out of 172 cases, 12 were hospitalized, or 6.97 percent. In Ohio, 96 of 1,300 cases resulted in hospitalization, equaling 7.38 percent. Health officials caution that hospitalization figures might exaggerate the risk for the average infected person, as many mild cases go undiagnosed.

Origins of Cyclospora

Cyclospora cayetanensis originates from human fecal contamination. Infection occurs after consuming contaminated food or water, especially fresh produce eaten raw. Past U.S. outbreaks linked to cilantro, basil, berries, and leafy greens. The parasite doesn’t spread directly from one person to another. After leaving the body, it needs environmental time before becoming infectious.

Although investigators found a lettuce-linked outbreak, officials say this doesn’t cover every illness this year. Multiple clusters remain under investigation. The U.S. Food and Drug Administration (FDA) continues searching for more contaminated products. In North Carolina, parsley and cilantro are suspects alongside lettuce.

Cyclospora Symptoms and Treatment

Symptoms usually start about a week after exposure but can appear within two days to two weeks. The primary symptom is watery diarrhea, which can become severe. Other symptoms include stomach cramps, bloating, nausea, weight loss, fatigue, and loss of appetite. Without treatment, symptoms can persist for weeks, sometimes improving only to return later.

The standard treatment is the antibiotic trimethoprim-sulfamethoxazole, often known as Bactrim. Hydration is crucial because prolonged diarrhea can lead to significant fluid loss. Physicians say many patients improve rapidly once correctly diagnosed and treated.

Seeking Medical Attention

Some cyclospora infections resolve without treatment, but experts advise against assuming the parasite will vanish on its own. Dr. Tyler Evans, CEO and founder of Wellness Equity Alliance, highlighted that cyclospora is often missed during testing.

“The people most at risk of staying sick are the ones whose clinician never orders the test,” he said. “Cyclospora does not show up on a standard stool panel. A clinician has to specifically ask for it.”

Evans noted that the immediate threat isn’t usually the parasite itself, but the dehydration resulting from prolonged diarrhea.

“What sends someone to the hospital is usually not the parasite itself. It is dehydration,” he said.

Experts recommend seeking medical care for persistent diarrhea, signs of dehydration like dizziness or reduced urination, ongoing vomiting, worsening symptoms, or symptoms not improving over time. People with weakened immune systems should also quickly contact a healthcare provider if symptoms arise.

The Severity of Cyclospora

Experts assert that death from the parasite is rare. “There have been no deaths in this outbreak,” Evans told Newsweek. “For most healthy people, this is a miserable illness, not a dangerous one.” CDC data supports this. Despite many cases nationwide, no deaths have been reported.

Evans said immunocompromised patients, including those with HIV, transplant recipients, and individuals undergoing cancer treatment, are at higher risk of severe illness. Still, he emphasized that cyclospora is generally treatable.

“I would not tell the public this is life-threatening,” he said. “I would tell them it is treatable. The danger comes from letting it go untreated for weeks because nobody ordered the right test.”

For most patients, doctors advise staying hydrated, seeking medical care if symptoms persist, and asking about cyclospora testing if severe diarrhea continues without a clear reason. Early diagnosis and treatment are key to preventing complications.
