Exercise and Metabolic Syndrome: Tailored Approaches Required

The fitness industry often debates which exercise is most beneficial. Running, weight training, HIIT, or yoga? A new study suggests that for those with metabolic syndrome, this may be the wrong question. The focus should be on how different exercises affect various aspects of the condition.
Published in iScience, the study proposes that exercise recommendations could be tailored to specific health concerns. Metabolic syndrome involves risk factors like high blood pressure, high blood sugar, excess waist fat, high triglycerides, and low HDL cholesterol. These increase the risk of cardiovascular disease, stroke, and type 2 diabetes.
While exercise is beneficial, doctors face challenges in recommending the best type for specific metabolic problems. For example, lowering blood pressure may require a different workout than reducing abdominal fat or improving cholesterol.
Researchers reviewed clinical trials on adults with metabolic syndrome, analyzing 53 randomized controlled trials with 2,948 participants. These studies covered aerobic exercise, resistance training, yoga, tai chi, qigong, and high-intensity interval training (HIIT).
No single exercise type dominated. Instead, different styles showed unique benefits. A combination of aerobic exercise and resistance training was effective in reducing waist circumference and improving cholesterol and blood sugar levels.
Activities like tai chi and qigong, incorporating breath control and mental focus, improved HDL cholesterol and systolic blood pressure. These suggest lower-impact activities play a key role in metabolic health.
High-intensity interval training (HIIT) also varied in effectiveness. Longer sessions reduced body fat percentage, while shorter workouts lowered triglycerides, total cholesterol, and diastolic blood pressure.
The study suggests viewing metabolic syndrome as related health challenges, not a single problem. It highlights the condition’s rising prevalence, with 1.54 billion adults globally affected as of 2023. This leads to higher cardiovascular disease risk and premature death, along with links to other health issues such as nonalcoholic fatty liver disease and polycystic ovarian syndrome.
While medications address symptoms like high blood pressure or blood sugar, they don’t tackle the broader metabolic problems. Diet and exercise can influence multiple risk factors, but researchers caution that current evidence is not definitive. Differences in study design and participant groups complicate firm conclusions. Future trials should standardize conditions to better compare exercise approaches.
