Waking up from a sharp pain at dawn on a chilly February morning in 2023, I felt an intense ache in my side. This was a step up from the persistent pain I had been feeling for weeks, akin to a severe runner’s cramp. Shortly after arriving at the emergency room, a doctor informed me that a scan revealed multiple myeloma. This blood cancer leaves distinctive lesions on bones. I learned I had numerous lesions, fractured ribs, and a compression fracture in my spine. There is no cure.
Outside the ER, I sought clarity from Google, finding an outdated statistic suggesting a life expectancy of three to five years. Fortunately, medical advancements, such as thalidomide-derived drugs, have extended patient lifespans.
Initially considered unusable due to its history of causing birth defects, thalidomide was reintroduced for multiple myeloma under the name Revlimid. This drug, which costs about $1,000 per pill to patients but only 25 cents to produce, has become a major investment for its maker, Celgene. Despite its life-saving properties, its exorbitant price has left patients in financial distress. My experience reporting on healthcare costs made this even more astounding.
Celgene increased Revlimid’s price 26 times. But the full story reveals complex regulatory oversights and strategies Celgene used to keep prices high.
From Thalidomide’s Dark Past to Revlimid’s Present
The journey of thalidomide is surprising. Initially banned for causing severe birth defects, it found a new role in cancer treatment. This transformation owed much to pharmaceutical efforts and the persistence of professionals like Dr. Robert D’Amato, who investigated its potential to inhibit blood vessel growth necessary for tumor development.
Dr. D’Amato realized thalidomide’s previous side effects might relate to its ability to block blood vessel formation. Revlimid, a derivative, successfully altered this mechanism to treat myeloma, leading to its modern application and widespread acclaim.
Challenges and Costs of Revlimid
Revlimid became a blockbuster, generating sales beyond $100 billion. Its cost has placed unbearable financial burdens on many patients and highlights the persistent challenge of drug accessibility. The pharmaceutical system’s flaws, especially unchecked price increases, make life-saving drugs like Revlimid unaffordable for some.
Celgene’s tactics included leveraging patents and safety measures to delay generic competition. Federal agencies tried intervening but faced hurdles. Meanwhile, Celgene continued to profit, raising prices and influencing healthcare markets.
The Market Dynamics
Generic drugs typically reduce brand-name prices dramatically after a few years on the market. Celgene delayed this effect through strategic agreements, maintaining high Revlimid prices. Although Bristol Myers Squibb acquired the company, the pricing pattern persisted.
These decisions led to substantial profits and executive pay, while patients and healthcare systems bore the brunt of cost increases.
The Personal Toll
For patients like me, the reality includes ongoing treatment. Even when in remission, maintenance therapies are necessary, ensuring the condition is managed but never eradicated.
Revlimid helps many, though at a cost. It imposes financial strain and can worsen with its side effects. Patients weigh the benefits against these challenges, hoping for long-term relief amidst pricing debates.
The struggle over drug pricing continues, affecting individuals and the broader health system. I experienced a shift to a generic version, revealing minimal savings. The high cost of care lingers, underscoring the need for sustainable solutions in pharmaceutical pricing.
