Multi-State Cyclospora Outbreak: Health Officials Investigate

Across several states, hundreds have contracted a microscopic parasite causing severe diarrhea and vomiting amid a surge in illness clusters. Federal and local health officials are actively investigating. The Centers for Disease Control and Prevention (CDC) disclosed that at least 145 cases of cyclosporiasis were confirmed nationwide from May 1 to June 16, resulting in at least 20 hospitalizations. However, localized data suggests the actual number of infections is much higher, with at least 18 states reporting cases.
Michigan: The Infection Hotspot
Michigan stands out as a significant hotspot for the infection. The Michigan Department of Health and Human Services (MDHHS) confirmed managing a “large and growing outbreak,” reporting over 300 cases in the state as of Thursday. Typically, Michigan records only about 50 cases of cyclosporiasis annually. The outbreak is mostly concentrated in Southeast and Southern Michigan, with Monroe County experiencing the highest number of infections. Lenawee, Washtenaw, Wayne, Livingston, Shiawassee, and Jackson counties, as well as Detroit, have also reported significant clusters.
“Outbreaks of cyclosporiasis have been occurring across the United States and now here in Michigan,” said Dr. Natasha Bagdasarian, the state’s chief medical executive. “Due to the unusual number of cases identified in a short period, we expect more cases to be reported.”
New York Cases On the Rise
In New York, health officials have identified 107 cases since early May, according to local reports.
Understanding Cyclosporiasis
Cyclosporiasis is an intestinal illness caused by the microscopic parasite Cyclospora cayetanensis. The CDC explains that the parasite spreads through consumption of food or water tainted with contaminated feces. Symptoms, which often resemble the flu, can appear up to two weeks after infection. They include nausea, abdominal cramps, fatigue, and watery diarrhea. Though healthy individuals usually recover without medical treatment, vulnerable populations like older adults are at higher risk for severe illness. The infection is not directly passed between people.
The disease is treatable with antibiotics such as trimethoprim-sulfamethoxazole, available under brand names like Bactrim, Septra, or Cotrim. Public health officials stress the importance of rest and fluid intake to combat dehydration from diarrhea.
Current State of Infections
Federal health officials indicate no singular source links all cases, suggesting multiple regional clusters might be occurring simultaneously. The highest concentrations of illness have been reported in Michigan, New York, Illinois, and Texas. Other affected states include Alaska, Colorado, Connecticut, Florida, Georgia, Louisiana, Massachusetts, New Jersey, North Carolina, Ohio, Pennsylvania, Tennessee, Virginia, and Wisconsin.
The CDC highlights that none of the individuals affected before June 16 had traveled in the two weeks prior to their illness. Past outbreaks have been associated with various imported fresh produce, including raspberries, basil, snow peas, mesclun lettuce, and cilantro. The CDC collaborates with the FDA and local health bodies like the Michigan Department of Agriculture and Rural Development (MDARD) to identify the source of recent outbreaks.
“MDARD is dedicated to maintaining food safety across Michigan and is partnering with local health departments to trace the outbreak source and protect consumers,” stated Jennifer Bonsky, Director of the Human Food Division at MDARD. She emphasized the necessity of following essential food preparation guidelines like washing hands and produce thoroughly.
